Lincolnshire Council is looking for a healthcare provider to deliver NHS Health Checks to patients in the Navenby Practice Code Area. You'd be responsible for screening eligible patients (about 492 invitations per year) for cardiovascular disease risk, providing lifestyle advice, and identifying high-risk patients who need further care from their GP. Payment is activity-based — you get paid per invitation sent and per completed check, with bonus payments if you hit 80% of your target.
