Overview
Suffolk County Council is preparing to procure its Health Visiting, School Nursing and Family Nurse Partnership services in the Waveney area. The Family Nurse Partnership (FNP) is a voluntary home visiting programme for first time young mums, aged 19 years or under.
The FNP programme aims to enable young mums to have a healthy pregnancy, improve their child's health and development and plan their own futures and achieve their aspirations. The Health Visiting and School Nursing Services will ensure that all children and young people receive the full 0-19 Healthy Child programme offer, including universal access and early identification of additional and/or complex needs, which improve the health and well-being outcomes for the children and young people in Waveney.
It is also important that these services work in an integrated manner with all service providers for children, young people and families to support seamless service provision. The provider will deliver high quality services which improve the experience of all service users whilst identifying those families in need of additional support or who are at risk of poor health outcomes.
