Overview
- description of the services to be provided: OPMH+ in Somerset is the Discharge service to support recovery, reablement and assessment and is the route for supported discharges where a person is unable to return to their usual place of residence at the point of discharge from acute hospital. The Bedded pathway model ensures no assessment for ongoing care happens in an acute hospital setting but happens in an appropriate pathway bed.
This enables speedy discharge and prevents the overprovision of long-term care. This service is provided by a combination of commissioned providers and NHS therapist staff and is jointly funded through the Intermediate Care budget. - this notice is an intention to award a contract under the most suitable provider process: - the approximate lifetime value of the contract: £3,328,000 You should also specify: - this is an existing service; - this is an existing provider; - the dates between which the services are intended to be provided, if known - 11.11.2026 - 10.11.2030
