Integrated Dementia Support Model

NHS Norfolk and Suffolk Integrated Care Board (ICB) intends to procure an outcomes-focused Dementia Support Service that acts as a strategic neighbourhood partner across the dementia pathway within the Norfolk and Waveney locality. The service will contribute to improved population health outcomes, reduced health inequalities, increased community resilience and improved integration between primary care, community services, mental health, social care and the voluntary, community and social enterprise sector.

The procurement will support delivery of the ICB's Ageing Well and Population Health Management priorities, enabling people living with dementia and their carers to remain independent, connected and well for as long as possible. NHS Norfolk and Suffolk Integrated Care Board (ICB) is exploring the future procurement of a Dementia Support Service for the population of Norfolk and Waveney.

The proposed service will provide person-centred support across the entire dementia pathway, from prevention, early identification and diagnosis through to living well with dementia, advanced care planning and end-of-life care. The service will support people living with dementia, those concerned about cognitive decline, their families and unpaid carers, recognising the significant impact dementia has on individuals, communities and the wider health and care system.

The future service is intended to operate as a strategic neighbourhood partner within the Ageing Well domain and contribute directly to the Norfolk and Waveney population health management approach. The service will be expected to support improvements in population health outcomes, reduce health inequalities, strengthen community resilience and help people remain independent and connected to their communities for as long as possible.

The service will adopt a preventative and proactive approach that promotes early intervention, supports people to "live well" with dementia and helps avoid unnecessary escalation of need, crisis presentations and reliance on statutory service. The ICB is seeking a model that is fully integrated with neighbourhood health arrangements and works collaboratively across primary care, community services, mental health services, social care, housing, acute providers and the voluntary, community, faith and social enterprise (VCFSE) sector.

The service will be expected to act as a connector across local systems, enabling coordinated support and improving people's experience of navigating services and accessing support. The ICB is engaging with the market to explore innovative delivery models, partnership approaches and opportunities to maximise social value, community capacity and integration across the wider health and care system.

Providers may include organisations from the NHS, local government, voluntary, community, faith and social enterprise sector and partnership or consortium arrangements. The scope, contract values, service model and procurement approach remain under development and will be informed through this market engagement activity.

The ICB is particularly interested in engaging with organisations that can demonstrate experience of: • Delivering population health management approaches. • Supporting neighbourhood health and integrated care models. • Addressing health inequalities and improving access for underserved groups. • Developing community capacity and asset-based approaches. • Providing coordinated support across health, care and VCFSE sectors. • Delivering measurable outcomes for people living with dementia and unpaid carers. This Prior Information Notice is to seek provider opinion on a range a questions formed from a variety of sources to gain a better understanding around deliverability, provider capability, partnership requirements, mobilisation risks and the outcomes that should be reflected in the future service model.

The deadline for completed responses to this PIN is midnight on 25th August 2026 Additional information: The intention is that any subsequent procurement process will be undertaken within the Provider Selection Regime (PSR). Any related procurement process will be governed by the Health Care Services (Provider Selection Regime) Regulations 2023.

For the avoidance of doubt, the provisions of the Procurement Act 2023 do not apply to this process. The Authority may wish to introduce additional services during the lifetime of the contract ("Contract modifications").

Such contract modifications will be contemplated where additional requirements are similar and or complementary to the services already included in the contract, at any given time. Such expansion would be by the addition of services commissioned by the Authority as named in any Contract Notice. To this end, the Bidder should be aware that after the award of contract, there is a possibility that the successful Bidder may be offered a variation to the contract in accordance with the Health Care Services (Provider Selection Regime) Regulations 2023.
