Bedford Borough Council ('the Council') is inviting organisations to submit a tender for the provision of a Home from Hospital and Reablement Service. Additional information: The contracting authority considers that this contract may be suitable for economic operators that are small or medium enterprises (SMEs).
Support is offered on an assessed needs basis for up to six weeks until they have fully settled back at home. This helps to reduce the number of delayed discharges from Bedford Hospital and reduce referrals to high cost services such as residential care and domiciliary care packages. Option to extend for any period or periods of up to two (2) years in 12 month blocks, such extension to be exercised at the Council's sole discretion.
