The service will provide accessible, safe and high-quality general practice services in accordance with the requirements of the APMS contract, including the provision of routine and urgent primary care, clinical assessment and treatment, preventative and health promotion services, long-term condition management, referral and care coordination, and appropriate collaboration with other health and care services. The service also includes the provision of a dedicated gender dysphoria service for eligible patients, supporting the assessment, care and ongoing management of adults with gender dysphoria/gender incongruence within primary care.

This includes appropriate clinical assessment, treatment and monitoring, prescribing where clinically appropriate and within the agreed service scope, and liaison and coordination with specialist gender identity services and other relevant healthcare professionals. The provider will be expected to deliver the service in accordance with relevant national guidance, clinical standards and locally agreed pathways, and to work collaboratively with the wider health and care system to support continuity of care, access and health outcomes for the patient population.

The services will be delivered from Cressex Health Centre and Lynton House Branch Surgery and will include services provided to the registered practice population and, where applicable, patients accessing the gender dysphoria service in accordance with the agreed eligibility and referral arrangements. Thames Valley ICB has followed PSR Direct Award C for the contract to which this notice relates.

The contract has a duration of 10 years. The contract value is calculated by reference to the weighted patient population, using the applicable per-weighted-patient funding methodology for the APMS service.

The estimated annual contract value has been calculated using the most recent completed financial year as the baseline, with an annual value of £1,265,548. On this basis, the estimated total contract value over the 10-year contract term is £12,655,480.

The annual value will be subject to variation over the contract term in accordance with changes to the weighted patient population, applicable national/local funding arrangements and any other payments or adjustments provided for under the contract. The value stated is therefore an estimate based on the most recent financial year's position and is not a fixed annual payment for each year of the contract.

The contract period will commence on 01 April 2027. This procurement has been carried out by NHS South, Central and West Commissioning Support Unit (SCW) on behalf of the Commissioners.

Additional information: Evidence that Better Health Bucks have satisfied the existing APMS contract to a sufficient standard: the provider has delivered the existing APMS services in accordance with the contractual requirements and service specification and has provided the required documentation, information and policies. The provider has complied with the contractual requirements relating to the delivery and quality of primary medical services, patient access, safeguarding, clinical governance, information governance, reporting and performance monitoring, and has met the applicable requirements for invoicing and payment.

The provider has also complied with the relevant NHS requirements and applicable legislation, policies and standards associated with the delivery of the APMS services, including requirements relating to safeguarding, quality and patient safety, information governance and cooperation with the Commissioner and other health and care partners. The provider has delivered the existing services to a satisfactory standard and there is no evidence of material or persistent contractual non-compliance that would indicate that it is unable to continue to deliver the services to the required standard.

The proposed contracting arrangements are substantially consistent with the existing APMS arrangements and the nature and scope of services currently delivered by the provider. Accordingly, the Commissioner is satisfied, based on the provider's performance under the existing contract and the evidence available, that Better Health Bucks is likely to continue to deliver the services to the required standard under the proposed contract. The Commissioner's assessment has taken account of the provider's performance throughout the current contractual period, including relevant quality, performance, governance and contractual information available to the Commissioner.
