Social Workers (Hospital Team) Rate of Pay: ?28.00 Ltd Hours: 37 Hours Location: See location details in advert below Start Date: ASAP Contract type: Agency - 12 weeks Job Description and Personnel Specification are attached, which also provides more information above the role. Sandwell Adult Social Care Health and Well-being are recruiting a Social Worker.
This is a borough wide hospital/intermediate care service. The service aims to improve the outcomes of residents in Sandwell by; o Enabling/facilitating timely discharges from hospital o Monitoring and facilitating timely discharges from intermediate care provisions o Offering alternatives to hospital admissions o Supporting people to return to independent living You will deliver an enhanced customer experience that promotes the wellbeing of vulnerable adults to enable them to live independently for longer, supporting them in retaining and regaining their skills and confidence.
You will be expected to manage a varied case load which includes access to Service Users who are COVID positive within the community within their own home, residential/Nursing care home settings and Hospital wards - to complete Face to Face assessments. You will be social work qualified and hold either the degree in social work, Dip SW, CQSW or equivalent and HCPC registered.

An enhanced DBS will be required for this role Applicants will be expected to be office and home based dependent upon a rota , if you are office based you will be required to work between 2 hubs which are Sandwell and Rowley Regis Hospital and will also be required to work out in the community as and when required. The agency workers will be provided with the following PPE, which includes Face shields, Gloves, Aprons and Hand sanitiser.
