Be headquartered at the Outreach provider’s office, but with the understanding that it is peripatetic/out in the field for a primary portion of the workday. Provide lived experience as core component of service provision.
Be expected to work 3 days per week and to meet with clients outside business hours as needed. Improve access to healthcare for people sleeping rough.

Be part of a whole-system approach that recognises the interdependence of health, housing and social inclusion. Improve uptake of outreach primary care offer.
Increase individuals’ confidence, knowledge and ability to take an active role in managing their own health and wellbeing, empowering them to engage effectively with treatment, appointments and longer-term health support. Maximise use of Health Emergency Bedspaces when available.
Support the implementation and evaluation of a City of London-led NHS information-sharing pilot. Improve early identification of long-term conditions that qualify people for support through health funded palliative care.
Support embedding inclusion health principles within outreach services, ensuring that health is a core component of their support. Amplify the voice of lived experience in homelessness work.
Play a key role in both housing stability and health outcomes for individuals identified as long-term rough sleepers, ensuring access to accommodation and services that meet their individual health and care needs. Work collaboratively with partner agencies and stakeholders to deliver the objectives set out in the MHCLG strategy and CoL’s Long Term Rough Sleeper partnership plan.
Management (including direct line management) of HAW workers will be the responsibility of the provider. The contract will run for an initial period of one year with the option to extend by one year. The maximum value of the contract will be £40,000 per annum.
