Overview

This notice is an Intended Approach Notice for the award of a contract for the delivery of NHS Health Checks Services in the Spilsby Practice Code Area, under the Provider Selection Regime, utilising the Most Suitable Provider Process. This notice will be published for 14 days before the authority proceeds to assessment of likely providers, as per the requirements of the Health Care Services (Provider Selection Regime) Regulation 2023.

The award decision makers will be: The Executive Director of Adult Care and Community Wellbeing. The new contractual arrangement will require the selected provider to deliver the key components of the NHS Health Check service, namely risk assessment, risk awareness and risk management.

Any additional testing or clinical follow up remains the responsibility for primary care, however there is an expectation that patients are provided with relevant lifestyle information and advice as part of the communication of the results of their health check. Those who complete the Health Check and are found to have a 20% or higher risk of getting cardiovascular disease over the next 10 years will move onto the high-risk register and will then be ineligible for the NHS Health Check.

Contact & helpdesk

Any potential provider who wishes to put themselves forward to be assessed as the Most Suitable Provider to deliver the NHS Health Checks Service in the Navenby Practice Code Area must email mark.fowell@lincolnshire.gov.uk, following which, the relevant documentation will be issued for completion to enable assessment.

Additional details

The total eligible Lincolnshire population for the NHS Health Check Programme is currently 2,461. Each eligible individual in the Navenby Practice Code Area is expected to be invited to receive an NHS Health Check once in every 5 year period, resulting in an annual invitation target of 492, or 41 per month.

Providers are paid according to activity, with a set cost per invite and per completed health check. There are also annual bonus payments available where Providers invite over 80% of their eligible target population each year and complete checks with incremental additional payments dependent on the proportion of checks completed to incentivise delivery.

The duration of the contract will be from 1st July 2026 to 31st March 2029. An extension period of up to 60 months is available within the current contract.

Medium Complexity

Lincolnshire Council is looking for a healthcare provider to deliver NHS Health Checks to patients in the Navenby Practice Code Area. You'd be responsible for screening eligible patients (about 492 invitations per year) for cardiovascular disease risk, providing lifestyle advice, and identifying high-risk patients who need further care from their GP. Payment is activity-based — you get paid per invitation sent and per completed check, with bonus payments if you hit 80% of your target.

Requirements

  • Must be able to deliver risk assessment, risk awareness, and risk management components of NHS Health Checks
  • Must invite and conduct checks with patients in the Navenby Practice Code Area
  • Must provide lifestyle information and advice to patients following their health check results
  • Must identify and refer patients with 20% or higher 10-year cardiovascular disease risk to the high-risk register
  • Must operate under the Health Care Services (Provider Selection Regime) Regulation 2023
  • Must be assessed as the 'Most Suitable Provider' by Lincolnshire Council's Executive Director of Adult Care and Community Wellbeing
  • Must meet expected annual invitation target of 492 eligible patients (41 per month) in the Navenby Practice Code Area

Key Tasks & Deliverables

  • Send invitations to approximately 492 eligible patients per year in the Navenby Practice Code Area
  • Conduct NHS Health Check appointments (risk assessments) with responding patients
  • Assess individual cardiovascular disease risk using standard NHS Health Check methodology
  • Communicate results and provide personalized lifestyle advice and information to each patient checked
  • Identify and flag patients with 20% or higher 10-year cardiovascular disease risk for high-risk register referral
  • Refer identified high-risk patients back to primary care (GP) for clinical follow-up
  • Maintain records and report activity data (invitations sent and checks completed) for payment and performance tracking

How to Read This Tender

  • 1This is an 'Intended Approach Notice' — it's a 14-day announcement before they formally assess providers. If you're interested, email mark.fowell@lincolnshire.gov.uk immediately to request the full tender documents and get on the assessment list.
  • 2Look for the 'Provider Selection Regime' rules — this is a different process from standard competitive tendering. It's designed to find the 'Most Suitable Provider' rather than the cheapest. Read the Health Care Services (Provider Selection Regime) Regulation 2023 to understand how you'll be assessed.
  • 3Payment is activity-based, not a fixed lump sum. You earn money per invitation sent and per completed check. There are bonus payments if you exceed 80% of your annual target (about 394 invites). Work out the unit costs from the tender documents and calculate realistic income based on expected completion rates.
  • 4The contract runs from July 2026 to March 2029 (nearly 3 years), with an optional 60-month extension available. Check whether the extension terms are guaranteed or discretionary — this affects your long-term planning.
  • 5Note that you're not responsible for additional clinical testing or follow-up — that's the GP's job. Your role is screening, communication, and referral. Don't assume you'll be doing extensive clinical work or ongoing management.

Tips for Small Businesses

  • Consider partnering with a local GP practice or primary care network — they may already have relationships with Lincolnshire Council and can help you understand the patient population and local pathways. You could subcontract delivery to them or form a consortium.
  • Work out your break-even point carefully. With an annual target of 492 invitations (41/month), you need to know the unit cost per invite and per completed check. If completion rates are lower than expected, you could lose money. Ask about historical completion rates in similar areas.
  • The bonus payments (for exceeding 80% of target) could significantly boost your income — hitting 394+ invites per year is your real income goal. Factor in the cost of chasing non-responders and whether you have the capacity to do so efficiently. Small businesses often struggle with outreach — consider whether you have the systems in place.
