Overview
The Metropolitan Borough Council of Sandwell is seeking tenders from suitably experienced and competent Providers of a NHS Health Check Service. The NHS Health Check programme aims to prevent heart disease, stroke, type 2 diabetes and kidney disease, and raise awareness of dementia both across the population and within high risk and vulnerable groups.
In April 2013, the NHS Health Check became a statutory public health service in England. Local authorities are responsible for making provision to offer an NHS Health Check to eligible individuals aged 40-74 years once every five years.
The council therefore require a contractor to identify suitable clients to receive a Health Check and provide an accessible service for clients to attend where they will then receive a Health Check delivered by the contractor. Following the Health Check the client will then receive tailored face-to-face feedback for each individual on their future risk of cardiovascular disease.
