Eligibility & compliance
Bidders are required to respond to questions directly within In-Tend and attach any requested documents separately to the placeholders provided.
Additional details
This includes specifically active management of long term and chronic conditions: patient referral, engagement and liaison with supplementary services where available routinely within the area, including specialist mental health services, drug and alcohol services and those available through secondary services. As it is likely that some of the patients on the VPS will have or have had a history of substance misuse, provider experience in this is essential, as well as having good working relationships with local specialist teams for onward referral and support to patients for rehabilitation.
