Overview
The Integrated Wellness Support Service (IWS) is a proposed model designed to improve health outcomes and reduce inequalities across Bolton by combining specialist stop smoking support and adult weight management into a single, holistic service. It will provide a single-entry point for residents seeking lifestyle and behavioural support, while also addressing wider determinants of health such as emotional wellbeing.
The service will adopt a household approach, working in partnership with local neighbourhood models and community assets to deliver flexible, person-centred interventions. Delivery will include community-based and digital options, ensuring accessibility and alignment with Bolton's vision to improve health and wellbeing for all age groups.
The service will deliver three levels of support: Universal Offer includes community-based health promotion, healthy eating guidance, physical activity initiatives, emotional wellbeing support, and signposting to local resources. It aims to build health literacy and encourage self-care strategies.
