Overview
Barnsley Metropolitan Borough Council is seeking to appoint a Provider to deliver an Integrated Carers Support and Prevention Service. The Service will provide support for unpaid carers aged 18 and over, together with low-level preventative support for adults aged 18 and over to help maintain independence, improve wellbeing and prevent, reduce or delay the need for statutory care and support services.
The Service will deliver an integrated model comprising carers support, carers financial advice and crisis support, prevention support for vulnerable adults, and community connection and navigation. The successful Provider will work collaboratively with Adult Social Care, health partners, neighbourhood teams and the voluntary, community and social enterprise sector to identify need early, provide timely support, improve wellbeing, reduce social isolation and prevent escalation into crisis.
The Contract will run for an initial five-year term, with an option to extend it by up to 24 months. The Council’s budget for the contract is £360,000 per year.
