There is a potential additional 2 year extension beyond this initial term. This service was previously commissioned as part of the Council and ICB's Community Services Prime Provider contract. The Council are commissioning these services directly as a new service and all GPs are new providers (whilst they are likely to have delivered the services under the previous commissioning arrangements).
