Digital Diabetes Prevention and Primary Hypertension Programme

NHS Scotland intends to establish a national Framework Agreement for the provision of a Digital Diabetes Prevention and Primary Hypertension Programme. The programme will support individuals identified as being at risk of developing Type 2 Diabetes and/or those diagnosed with primary hypertension, through the delivery of evidence-based, digitally enabled lifestyle and behaviour change interventions.

The Authority intends to appoint a single supplier to a national Framework Agreement. The procurement will be conducted using a Restricted Procedure, comprising a Selection Stage and an Invitation to Tender (ITT) stage.

Suppliers will be shortlisted following evaluation of their suitability, technical and professional capability, capacity and relevant experience, with a maximum of five suppliers invited to submit a tender. NHS Scotland intends to establish a national Framework Agreement for the provision of a Digital Diabetes Prevention and Primary Hypertension Programme.

The Framework will support NHS Scotland's strategic objectives to improve population health outcomes, reduce health inequalities and support the prevention and management of long-term conditions through evidence-based, digitally enabled interventions. The requirement comprises a single integrated service with two pathways: - Digital Diabetes Prevention Programme (DDPP) for individuals identified as being at increased risk of developing Type 2 Diabetes; and - Primary Hypertension Programme (PHP) for individuals diagnosed with primary hypertension, supported through lifestyle and behaviour change interventions designed to improve health outcomes and reduce cardiovascular risk.

The appointed Framework Participant will be required to deliver evidence-based, digitally enabled lifestyle and behaviour change interventions designed to support participants in achieving sustainable improvements in health behaviours and clinical outcomes. The service will utilise a digital platform/application supported by appropriately trained personnel, including health coaching and clinical governance arrangements, to facilitate participant engagement, self-management, education, monitoring and behaviour change.

The service is expected to support a range of participant needs and will include, as appropriate, digital onboarding, risk assessment, lifestyle intervention, coaching support, educational resources, progress monitoring, outcome reporting and onward referral or escalation processes where clinically indicated. The Authority seeks a solution that is accessible, inclusive and capable of supporting diverse populations across Scotland, including individuals who may experience barriers to participation arising from geography, deprivation, language, disability or digital exclusion.

The Framework Participant will be expected to work collaboratively with NHS Scotland, NHS Boards, Primary Care and other relevant stakeholders to support the promotion of the programme, participant recruitment and self-registration, service mobilisation, programme delivery, performance monitoring and continuous improvement. Current funding has been identified to support approximately 15,000 participants through the Diabetes Prevention pathway during the initial three-year term of the Framework Agreement.

The Hypertension pathway is anticipated to support approximately 500 participants per annum. These figures are provided for information only and do not constitute a guarantee of volumes.

The Authority reserves the right, subject to future funding availability and service requirements, to increase participant volumes and/or expand the scope of the Framework to include additional prevention cohorts, pathways, clinical indicators or service requirements that are closely aligned to the subject matter of the Framework Agreement. The Authority intends to appoint a single supplier to a national Framework Agreement.

The procurement will be conducted using a Restricted Procedure. Suppliers will first be assessed at Selection Stage through completion of the Single Procurement Document (SPD) and Pre-Qualification Questionnaire (PQQ), which will assess suitability, technical and professional capability, capacity and relevant experience. A maximum of five suppliers will be shortlisted and invited to participate in the Invitation to Tender stage.
