Additional details

Scope of Services: The commissioned services will provide intensive, enabling support to residents to regain the skills, confidence and social networks to return to their previous levels of independence as far as possible and reduce the likelihood of admission/ re-admission to hospital, or the need for long term care and support. While many people will not need support after reablement, some people will require further support to enable them to continue to live safely at home.

The new model will take a more enabling approach to homecare, thereby increasing opportunities for further independence through closer relationships with our community health and primary care services to ensure early intervention when there is any deterioration or concerns with health conditions. Lot 1 focuses on short-term, intensive reablement support for adults to help them regain independence following illness, hospital discharge, or a crisis.

The service is time-limited and outcome‑focused, supporting people to relearn daily living skills, reduce reliance on ongoing care, and maximise independence within their own homes. The emphasis is on strengths‑based practice, multidisciplinary working, and preventing or reducing long‑term care needs.

Lot 2 covers longer-term homecare support for residents who require ongoing assistance to live safely at home. This includes personal care and daily living support delivered in a person‑centred and flexible way, aligned with assessed needs and care plans. The service aims to maintain independence, stability, and wellbeing, while supporting hospital avoidance and timely hospital discharge where appropriate.
