Overview
The proposed Community Life Choices (CLC) Framework is intended to support adults aged 18 and over who are eligible for social care support following assessment under the Care Act 2014. The framework is expected to provide services that promote independence, wellbeing, community engagement and positive personal outcomes, enabling people to live as independently as possible within their local communities.
This includes group support and one-to-one support provided in the community from a building base. Examples of one-to-one support include travel training, support with work and volunteering opportunities.
Group support can include visiting venues and groups in the local community and further afield. Within the building base and in the community, this may include the delivery of personal care
