This urgent award is to provide Independent social workers, occupational therapists and nurse practitioners working alongside All Age Continuing Care ( AACC) to complete assessments for equipment for Continuing Health Care ( CHC) eligible individuals in Cheshire West. NHS Cheshire and Merseyside ICB - Cheshire West Place intend to award a 6 month contract under the grounds of urgency to provide assessments and equipment for patients eligible for All Age Continuing Healthcare ( AACC).
