Wiltshire Council – Home from HospitalMarket Engagement Event InvitationWiltshire Council is pleased to invite interested providers to a market engagement event ahead of the procurement of a new Home from Hospital contract, with the intention to award the contract in Spring 2027.About the Home from Hospital ServiceThe Home from Hospital service is a highly collaborative and coordination-focused service providing, flexible practical support (not personal care) for a period of up to 4 weeks. The service is for vulnerable Wiltshire residents (aged 18+) who are medically fit for discharge but unable to return home due to practical barriers.
The service runs from Monday to Friday with flexibility to support over weekends when there are Hospital pressures.The service is responsive and works effectively with a range of partners to facilitate discharges. The successful service provider will have skills in navigating and coordinating solutions that prevent an individual being discharged home.
Contact & helpdesk
Tasks are likely to include:• Presence within each of the hospitals in Wiltshire (GWH, RUH and SFT) and the community hospitals • Meeting with individuals on wards to understand barriers to their return home and being innovative in co-producing resolutions• Supporting individuals to pay for services or goods to support their discharge home (food, locksmiths, deep clean services)• Signposting to advice and support around benefits and accessing emergency funding • Linking up with the individual with community support available and other services to resolve complex but practical issues (befriending, shopping, meal services, accessing property etc)• Home preparation before discharge, accessing property (with consent), removal of clutter, coordinating access for equipment deliveries and ensuring utilities are accessible (gas, electric and food)• Supporting individuals with building resilience upon their return home to remain independent longer termMarket Engagement Event DetailsThis engagement event will:• Provide an overview of the upcoming contract• Outline the indicative procurement timeline and anticipated procurement route• Offer interested providers the opportunity to learn more about the service and share views on proposed future delivery models• Include time for questions and discussionEvent Format: Microsoft Teams• Date: Thursday 3rd September 2026• Time: 11:00am – 11:50am• Location: Microsoft Teams (joining link provided upon registration)How to RegisterTo register your interest and receive the joining link, please email Adults Commissioning at: adultscommissioning@wiltshire.gov.ukPlease include the following text in the subject line:Home from Hospital market engagement event 03/09/2026Registration deadline: Wednesday 26th August 2026
