Overview
This urgent award is to provide Independent social workers, occupational therapists and nurse practitioners working alongside All Age Continuing Care ( AACC) to complete assessments for equipment for Continuing Health Care ( CHC) eligible individuals in Cheshire West. NHS Cheshire and Merseyside ICB - Cheshire West Place intend to award a 6 month contract under the grounds of urgency to provide assessments and equipment for patients eligible for All Age Continuing Healthcare ( AACC).
This presents a significant patient safety risk. Without timely assessment and provision of necessary equipment, there is a high likelihood of deterioration in patients' conditions, which may lead to requirement of increased carer support, avoidable hospital admissions and increased pressure on acute services.
The Contract was awarded on 3rd May 2026 and will conclude on 30th September 2026, the contract value is £23,000. This is to address immediate service gaps and patient safety risks following withdrawal of services, whilst a review is completed to determine future commissioning arrangements.
